LafayetteCultural Arts Commission Arts in the Community Grant Application
Application Deadline: Grants will be accepted from March 1, 2012 – April 18, 2012.
A 5-10 minute interview/presentation may be required. If so, applicants will be notified. All applicants will be contacted by mid-May regarding the status of their application.
The Lafayette Cultural Arts Commission (LCAC) promotes, nurtures and celebrates the visual, performing and language arts in Lafayette, CO by:
1) Providing opportunities for the public to experience and enjoy art and culture in its various forms
2) Connecting artists, educators and community members with resources
3) Building partnerships to sustain and further arts and culture
4) Embracing and supporting the diversity of the community we serve
General Requirements for the Arts in the Community Grant
1. All applicants must be of legal contractual age as defined by the state of Colorado (18+).
2. All projects must directly benefit the city and residents of Lafayette.
3. Projects should be completed by 04/1/2013.
4. A final report will be completed by the grant recipient by April 10, 2013. The final report form will be provided to all grant recipients at the time the grant is awarded.
5. Credit must be given to LCAC on all promotional materials and at any event associated with the grant project.
6. Photographs of recipients and project may be used by LCAC to support its mission.
General Application Guidelines
Eligibility:Project-based grants are available to groups or individuals who qualify under the General Requirements.
Awards are unlikely to exceed $1000. There is no restriction as to the minimum amount that may be requested.
For Individuals:The State of Colorado law requires proof of citizenship at the time of the grant award.
Exclusions:Funds will not be provided for: Projects that have occurred before the grant deadline, capital improvements, fundraising events, receptions, existing general operating budgets and/or salaries for employees of organizations, reductions of deficits from past programs.
Evaluation of the Application:Each grant will be evaluated on the following:
Project quality
Budget
Community impact
Demonstrated need for support
Types of programs and services offered
Completing the Application:Applications must be typed. Answer all questions concisely using no more than ¼ page for each answer. You may request a word document of the application from susanb@cityoflafayette.com.
Submission:Submit the completed application to susanb@cityoflafayette.com. Only electronic submissions of the form will be accepted. Video and audio materials may be linked for online viewing (not having online video or audio will not count against you). If you have supplementary materials such as photographs, drawings, press cuttings, etc. mail them to:

B. Project Description
Submit up to a two-page narrative that responds directly to the following. Be clear and concise. Number, repeat the questions, and answer in order. Keep in mind the evaluation criteria. Satisfactory reporting and documentation, in addition to a written assessment of the program will be required upon completion of the grant.
Organizations:Organizations are encouraged to submit proposals for new or enhanced projects OR to become partners with other organizations to develop new projects.
Individuals:The Commission is interested in work that is innovative, collaborative and that broadens the scope of the community’s experience of art.
FOR ORGANIZATIONS
1. Briefly describe what you propose to do, how you plan to accomplish it, and why your project merits funding.
2. Describe community involvement, the anticipated community impact, and public access to project activities. Include any outreach to underserved population.
3. Describe your organization; include structure, mission statement, vision statement (if applicable), brief history, programs, services, and community you serve.
FOR INDIVIDUALS
1. Briefly describe what you propose to do, how you plan to accomplish it, and why your project merits funding.
2. Describe community involvement, the anticipated community impact, and public access to project activities.
3. Please attach a current resume and an artist statement.
